Errors and states
Every error carries a code, a message, the offending param and a request_id for support. Only quote_expired and provider_unavailable are worth retrying; validation, balance and policy errors need a fix first.
Codes

| 400 validation_error | A field is missing or malformed; param names it |
| 400 channel_unavailable | Channel not live for your account or country |
| 400 amount_out_of_range | Below channel minimum or above maximum |
| 401 unauthorized | Missing or invalid key |
| 403 verification_required | Bank funding or bank payout before business verification |
| 403 policy_exceeded | Key budget, channel list or approval threshold exceeded |
| 404 not_found | Unknown id |
| 409 idempotency_conflict | Same key, different body |
| 409 approval_required | Launch attempted with approvals outstanding |
| 410 quote_expired | Quote older than 60 seconds |
| 422 recipient_not_ready | Recipient is missing fields for this channel |
| 429 rate_limited | Slow down; see Retry-After |
| 502 provider_unavailable | Upstream rail error; safe to retry with the same key |
